The soluble receptor for advanced glycation end-products (sRAGE) has antiinflammatory properties, and deficiency of circulating sRAGE is associated with various human diseases. Whether sRAGE concentrations are reduced in chronic obstructive pulmonary disease (COPD) has not been determined. The aim of this study was to determine plasma levels of sRAGE in COPD patients and establish whether sRAGE varies in relation to forced expiratory volume in 1 s (FEV1) and other inflammatory markers.61 COPD patients and 42 healthy controls were recruited. Plasma sRAGE, C-reactive protein (CRP) and serum amyloid A (SAA) were measured in patients with stable COPD. A subgroup had measurements during acute exacerbations of COPD (AECOPD).sRAGE was significantly lower in stable COPD than in healthy controls (p,0.001), while CRP (p,0.001) and SAA (p50.015) were higher in stable COPD than in healthy controls. Multiple linear regression confirmed that COPD was negatively associated with sRAGE (p,0.001). Plasma sRAGE was positively correlated with FEV1 (r 2 50.530, p,0.001), while CRP and SAA were inversely proportional to FEV1. Multiple linear regression analysis showed that only sRAGE was a strong predictor of FEV1. AECOPD were associated with even lower sRAGE levels that increased with convalescence. Circulating sRAGE is lower in COPD and shows a strong correlation to the degree of airflow limitation.

ABSTRACT. Recent studies have revealed that the inflammatory process plays a role in the pathogenesis of osteoarthritis (OA). The S100 family and receptor for advanced glycation end products (RAGE) 11363 RAGE gene in osteoarthritis ©FUNPEC-RP www.funpecrp.com.br Genetics and Molecular Research 14 (3): 11362-11370 (2015) participate in regulating inflammation, even in the production of matrix metalloproteinases (MMPs). MMP-1 degrades cartilage, which may result in OA development. Moreover, polymorphisms in RAGE, S100A8, and MMP-1 have a marked effect on ligand binding and transcription regulating. In this study, we investigated the potential genetic contribution of the RAGE, S100A8, and MMP-1 genes to OA. We performed a matched case-control association study and genotyped OA patients and healthy controls, who were analyzed by polymerase chain reaction-restriction fragment length polymorphism assays. A total of 207 patients were diagnosed with knee OA and underwent total knee replacement. The control group included 207 individuals who had standard X-rays of the knee joints to confirm K/L < 2 and were matched by age and gender. Single-nucleotide polymorphisms in RAGE (-429T/C, -374T/A, and 557G/A), S100A8 (rs3795391A/G), and MMP-1 (-1607 1G/2G, -755G/T, and -519A/G) were evaluated. RAGE -374T/A, S100A8 rs3795391A/G, MMP-1 -1607 1G/2G, -755G/T, and -519A/G showed no significant difference between OA patients and healthy controls. RAGE -429T/C and 557G/A showed a significant association between OA patients and healthy controls (P = 0.016 and 0.047, respectively). In haplotype analyses, no RAGE and MMP-1 haplotypes showed associations with OA. Our results suggest that the investigated polymorphism in the RAGE gene play a role in OA in the Han Chinese population.

This thesis focuses on cardiovascular (CV) disease (CVD) risk in rheumatoid arthritis (RA). Inflammation and CV risk factors are increased in RA patients. This population has a higher carotid intima-media thickness and more CV events than the general population and kynurenine (KYN) concentration levels are higher in RA patients compared to healthy controls. Therefore, in this thesis inflammation and CVD risk factors are investigated for increased risk of future RA development. RA and type2 diabetes (T2D) carotid intima-media thickness (CIMT) are compared and measurements of serum KYN for CVD, cancer and mortality are studied. First I tested the hypothesis that inflammation, CVD and CV risk factors are associated with increased risk of future RA development. Data from the Norwegian HUNT population health survey were obtained to compare groups with RA at baseline and follow-up (HUNT2 and HUNT3, n = 429) or follow up alone (HUNT3, n = 786), or without RA at both times (n = 33,567). Results showed female gender, age, smoking, body mass index (BMI), and history of previous CVD were associated with self-reported incident RA (previous CVD: odds ratio 1.52 (95% confidence interval 1.11-2.07). The findings regarding previous CVD were confirmed in sensitivity analyses excluding participants with psoriasis (odds ratio (OR) 1.70 (1.232.36)) or restricting the analysis to cases with hospital diagnosis of RA (OR 1.90 (1.10-3.27)) or carriers of the shared epitope (OR 1.76 (1.134-2.74)). History of previous CVD was not associated with increased risk of osteoarthritis (OR 1.04 (0.86-1.27)). I then compared the characteristics of CV risk and progression of CIMT in 78 RA patients from an outpatient Rheumatology Clinic and 212 patients with T2D who attended a community or hospital diabetes clinic in Brisbane. We found that the burden of risk varied between the 2 cohorts.RA patients were older, had a higher proportion of smokers, females and previous CVD. T2D patients had a higher body mass index (BMI), diastolic blood pressure, triglycerides, lower high density lipoprotein and a higher statin use. At baseline, CIMT measurements were similar in the RA and T2D cohorts. In an adjusted linear regression model, RA was significantly associated with lower CIMT at follow-up. Despite a shorter follow-up, 91 % ii of the T2D cohort had increased CIMT at follow-up compared to 54 % of the RA cohort. In the RA cohort, disease modifying anti-rheumatic drug (DMARD) use at baseline was associated with significantly lower CIMT values at follow-up.Finally we hypothesized that increased KYN in RA patients may predispose to CVD, infections and cancer. We measured KYN in 129 RA patients followed for 10 years for development of CVD, cancer and death.
