Reconditioning by end‐ischemic hypothermic in‐house machine perfusion: A promising strategy to improve outcome in expanded criteria donors kidney transplantation

Abstract: This clinical study evaluates end-ischemic hypothermic machine perfusion (eHMP) in expanded criteria donors (ECD) kidneys. eHMP was initiated upon arrival of the kidney in our center and continued until transplantation. Between 11/2011 and 8/2014 eHMP was performed in 66 ECD kidneys for 369 (98-912) minutes after 863 (364-1567) minutes of cold storage (CS). In 49 of 66 cases, the contralateral kidney from the same donor was preserved by static CS only and accepted by another Eurotransplant (ET) center. Five (1… Show more

“…Whereas the treatment of brain dead donors has been addressed in only a small number of clinical trials (2931), conditioning of the organ by machine perfusion is currently considered in a clearly larger number of clinical trials (4, 6, 3234). These studies mainly address the impact of hypothermic vs. cold storage machine perfusion, whereas alternative approaches for organ conditioning are tested solely in pre-clinical models (3537) or have been proven insufficient (38).…”

“…The authors believe the pulsatile stimulation of the renovasculature is an important reason for these results. It might preserve the vascular endothelium and its phenotype, preventing transcription of inflammatory and vasoconstrictors factors …”

“…The New York City OPO uses a hybrid strategy for kidneys allocated from a distant procured donor hospital (imported kidneys); these kidneys arrive after a mean CIT of 32 hours in preservation solution and ice and are connected to the MP when they arrive in New York City . However, the employment of machine perfusion following static cold storage vs the use of machine perfusion throughout the entire preservation period is still a matter of debate and recently has been investigated …”

“…11 Static cold storage (SCS) alone is the worldwide "gold standard" for organ preservation. 17 More recently, end-ischemic strategies have been shown to decrease the incidence of DGF by using HMP 18 or normothermic perfusion (NP) 19 after a period of SCS compared with SCS alone. 12,13 Continuous hypothermic machine perfusion (HMP) started immediately after procurement until implantation demonstrated better transplant outcomes compared with SCS alone [14][15][16] and reduced the risk of DGF regardless of donor type and cold ischemia time.…”
