Reconstruction of the face by means of the angle-rotation flap

Abstract: This article deals with defects in the face. A cervical flap called the angle-rotation flap was created by Schrudde 16 years ago to cover defects such as scarred skin after deep burn injuries as well as skin and subcutaneous defects following removal of dysplastic skin or after accidents. Indications and the method are described.

“…This flap has been traditionally used to cover large defects in the lower eyelid and cheek. 2,3 As noted before, the use of this flap to cover a defect affecting the upper lip was also described by Pribaz. 2 As the aesthetic junction of the cheek to upper lip is violated by this technique, the nasolabial depression can be created at the second-stage operation by debulking the area of the nasolabial fold as well as the alar-facial junction.…”

“…The Schrudde flap has the advantage of supplying a large area of cheek and neck skin to reconstruct defects of the cheek while not leaving any scars on the neck and chest, and it may be readvanced in future reconstruction, especially well noted by Joel Feldman (B. Zide, personal communication). 2,3 The cheek and neck skin provide an ideal color and texture match for cheek defects. The resulting scars lie in the aesthetic unit borders of the cheek with the upper lip, nose, lower lid, and ear.…”

“…(Fig 6). Schrudde and Beinhoff 19 have described the use of the angle-rotation flap for cheek reconstruction. This flap can be reused multiple times to resurface the entire cheek or can be used in conjunction with tissue expansion.…”

“…Classic cheek flap was modified several times by different surgeons. In one of these modifications, Schrudde described an angle-rotation flap, which extended the flap design behind the ear at a sixtyto-ninety-degree angle, mobilising retroauricular tissues for better coverage [19]. For coverage of more medially located defects, modifying this flap design by extending incision from retroauricular to upper cervical tissues with a curved incision (as a back-cut), rectified this problem [20].…”
