Reconstruction of Traumatic Medial Malleolus Loss With a Free Iliac Crest Autograft: Case Report

Abstract

The aim of the study is to demonstrate a rare clinical case of a recurrence of HO in patient after a primary total hip arthroplasty, accompanied by ankylosing
 Case presentation. A 52-year-old patient received a motorcycle injury, as a result, open fracture of the medial ankle with the loss of a bone fragment. The patient was taken to a medical institution, where primary surgical treatment with wound suturing. After 3 months, the patient was sent to the Priorov National Medical Research Center of Traumatology and Orthopedics, where an operation was performed to reconstruct the medial ankle with a free autograft from the iliac crest. Autograft was fixated with a metal structure and plastic of the deltoid ligament. In the postoperative period, immobilization of the ankle joint was performed for a period of 4 weeks, then active development of movements and dosed load on the limb after 8 weeks. After 12 months, the results of the clinical assessment on the AOFAS scale were 93 points. According to computer tomography, complete integration of the autograft was achieved and there were no signs of instability of the ankle joint. The patient is satisfied with the surgical treatment performed
 Conclusions. The most optimal, in our opinion, is the reconstruction from the iliac crest, since this allows the formation of a graft of the necessary parameters and shape, while minimizing the risk of postoperative complications.
