Rectal Dieulafoy’s lesion: a comprehensive review of patient characteristics, presentation patterns, diagnosis, management, and clinical outcomes

Abstract: Dieulafoy's lesion is an abnormally large, tortuous, submucosal vessel that erodes the overlying mucosa, without primary ulceration or erosion. Although these lesions predominantly involve the stomach and upper small intestine, they are being detected with increasing frequency in the rectum. We conducted a systematic literature search of MEDLINE, Cochrane, Embase, and Scopus databases for adult rectal Dieulafoy's lesion. After careful review of the search results, a total of 101 cases were identified. The data… Show more

“…83 Hemoclip application via device-assisted enteroscopy has gained preeminence in the current times. 84 -88 In our review, endoscopic hemoclipping alone achieved a relatively higher primary hemostasis rate (92%) and a lower rebleeding rate (8%). Endoscopic band ligation is relatively easier than hemoclipping as its versatility allows it to reach in difficult gastrointestinal sites.…”

“…81 Mechanical endoscopic methods such as hemoclipping and band ligation confer a slight therapeutic superiority over other endoscopic techniques. [82][83][84] Recently, hemoclipping is exacting to come to terms within locations such as the small bowel (Figure 5). 73,82 Major predictors of successful hemoclipping include the presence of surrounding soft tissue and appropriate positioning of hemoclips.…”

“…Professor Georges Dieulafoy had described Dieulafoy arteriole a century and half ago, yet, the pathogenesis and the mechanism of bleeding from this vascular anomaly had not been precisely appreciated [7]. Minor trauma to a point in the mucosa which is pressurized by the enlarged dilated arteriole has been postulated as the likely mechanism for a bleeding Dieulafoy arteriole [13].…”

“…Dieulafoy arteriole is a very infrequent lesion mostly appearing in the lesser curvature of the stomach [1]. Many reports described Dieulafoy lesions in other parts of the gastrointestinal tract [2][3][4][5][6][7].…”
