Background: Coital trauma cases reported in the literature mostly occur in the context of sexual violence, abuse, or rape. Consenting sexual intercourse is exceptional as the origin of a rectovaginal fistula. Such a fistula might be caused by the woman's virginity or sexual abstinence, the disproportion in size between the penis and vagina, or by vaginal dryness and the repeated aggressive use of foreign objects. Case: The case of a 22-year-old African woman from Senegal who was treated for rectovaginal fistula that had occurred in the context of consenting sexual intercourse, is presented. The couple did not have any anomaly or genital malformation. The patient was admitted 2 days after her first sexual intercourse for vaginal bleeding and release of fecal matter through vaginal route. Examination of the perineum showed a low rectovaginal fistula that was treated by a one-time repair. The patient had an uneventful postoperative course and was discharged 5 days after the surgical operation. Conclusions: Rectovaginal fistula after consenting sexual intercourse is exceptional. Virginity and lack of sexual education were the main catalyzing factors in the case reported here. ( J GYNECOL SURG 28:292)
