Recurrence of hydrosalpinges after cuff neosalpingostomy in a poor prognosis population

Abstract: Purpose: The purpose of this study was to determine the recurrence rate of hydrosalpinges after cuff neosalpingostomy in poor prognosis candidates. Methods: Forty consecutive patients with hydrosalpinx treated with cuff neosalpingostomy were included. Main outcome measures were recurrence rate of hydrosalpinx after cuff neosalpingostomy, intrauterine and ectopic pregnancy rates. Results: Intraoperatively, the mean size of the hydrosalpinx was 1.9 ± 0.7 cm, and 77% of patients had evidence of pelvic adhesions. “…The pooled clinical pregnancy rate of these 12 studies was 31%. One study published by Bayrak et al (2006) had a higher mean age (35.5 years) and had the lowest clinical pregnancy rate (5%). This may suggest that salpingostomy and waiting for natural pregnancy are not a suitable management strategy in older women with hydrosalpinx.…”

“…However, serious concerns remain that a restored tube could expose the patient to a high, unnecessary risk of ectopic pregnancy [ 12 ]. According to Bayrak et al , functional repair of hydrosalpinx in adult patients treated for infertility by laparoscopic neosalpingostomy is not recommended because of the dismal spontaneous pregnancy rate and an up to 70% incidence of recurrence of hydrosalpinx, which requires a second surgery before IVF [ 13 ].…”

“…Alternatively, some surgeons apply bipolar energy to the serosa to contract the serosa and thereby evert the cut ends without placing sutures. The increasing success of IVF combined with evidence for a high rate of recurrence in poor prognosis population has seen this surgery becoming almost obsolete [41].…”
