Recurrent cerebral attack caused by thrombosis in the pulmonary vein stump in a patient with left upper lobectomy on anticoagulant therapy: case report and literature review

Abstract: BackgroundThrombus formation in the pulmonary vein stump after pulmonary resection has recently been identified as a cause of systemic thrombosis including brain infarction. However, there is limited research focusing on the clinical course of pulmonary vein stump thrombus, and optimal treatment and prevention strategies of this important complication have not been established.Case presentationA 77-year-old woman was diagnosed with lung cancer of the left upper lobe, cT4N2M0, cStage IIIB. As the tumor was cons… Show more

“…So far, 13 cases of embolic stroke with thrombus in the pulmonary vein stump (Table 1) were reported previously. 515) Two cases were transient ischemic attack and 11 were cerebral infarction. In all cases, lung resection involving the left upper lobe were performed for malignant lung tumor.…”

“…A thrombus in the pulmonary vein (PV) stump is a rare but important complication after lobectomy, because it may cause systemic embolization. [1][2][3][4][5][6][7] According to previous reports, embolisms caused by this complication can occur in vital organs including the brain, kidneys, and intestines. [1][2][3][4][5][6][7] It was reported that a left lobectomy might be a risk factor for atrial fibrillation, which can then lead to thrombi in the systemic circulation.…”
