Redo surgery risk in patients with cardiac prosthetic valve dysfunction

Abstract: IntroductionThe aim of the study was to analyse the risk factors of early and late mortality in patients undergoing the first reoperation for prosthetic valve dysfunction.Material and methodsA retrospective observational study was performed in 194 consecutive patients (M = 75, F = 119; mean age 53.2 ±11 years) with a mechanical prosthetic valve (n = 103 cases; 53%) or bioprosthesis (91; 47%). Univariate and multivariate Cox statistical analysis was performed to determine risk factors of early and late mortalit… Show more

“…Patients presented with valve obstruction and low blood pressure are prone to kidney injury which further leads to fluid retention and exaggerates the heart failure. In agreement with other studies, the previous stroke was associated with higher mortality [20]. Stroke can occur due to concomitant carotid disease or embolization from left atrial thrombus.…”

“…Mortality in the current study was significantly associated with long CPB and cross-clamp times which is consistent with the published series [16,19,23]. The cause of the prolonged CPB time was the lengthy weaning from the circulatory support because of the associated poor left ventricular function [20] which is a risk factor for morality [24,25].…”

“…Additionally, both women were in poor biological condition. Moreover, redo procedures are always associated with higher risk [ 8 ]. On the other hand, some authors have proved that reoperation is not an independent risk factor for operative mortality.…”

“…Recently, Maciejewski et al. evaluated 194 patients undergoing redo valve surgery (aortic or mitral) and quoted a 30‐day mortality rate of 18.6% . The authors found that NYHA class III–IV and infective endocarditis were significant multivariable predictors of mortality.…”
