Reduced striatal dopamine transporters in idiopathic rapid eye movement sleep behaviour disorder

Abstract: Rapid eye movement (REM) sleep behaviour disorder (RBD) is characterized by complex behaviour during REM sleep. The aetiology of this disorder is still unknown, but a recent study showed an association between RBD and Parkinson's disease. We therefore studied striatal postsynaptic dopamine D2 receptor density with [123I](S)-2-hydroxy-3-iodo-6-methoxy-(1-ethyl-2-pyrrolidinylmethyl ) benzamide ([123I]IBZM) and the striatal presynaptic dopamine transporter with (N)-(3-iodopropene-2-yl)-2beta-carbomethoxy-3beta-(4… Show more

“…While there is no clear evidence that dopaminergics influence RBD symptoms (Iranzo et al, 2009a), imaging studies nonetheless indicate that dysfunction of the nigrostriatal dopamine system is associated with RBD (Albin et al, 2000;Eisensehr et al, 2000). We suggest that initial RBD onset is triggered by loss of GABAergic/glycinergic function and that progressive dopamine cell degeneration not only causes Parkinson's symptoms but also worsens RBD.…”

“…22,23 Of note, the PDRP network has several advantages compared with dopaminergic imaging techniques. While PDRP expression is associated with both presynaptic nigrostriatal dopaminergic dysfunction and Unified Parkinson's Disease Rating Scale motor ratings, 6,24 it is worth noting that the magnitude of these correlations is modest.…”

“…Using 123 I-IPT SPECT, Eisensehr et al found reduced striatal DAT binding in all 5 of their patients with idiopathic rapid-eye-movement sleep behavior disorder (64). In another series, 11 patients with sleep disorders were investigated with FP-CIT SPECT, and reduced striatal DAT binding was found in 3, 1 of whom had evidence of clinical parkinsonism (65).…”
