Reduced Susceptibility to Azithromycin and High Percentages of Penicillin and Tetracycline Resistance in Neisseria gonorrhoeae Isolates from Manaus, Brazil, 1998

Abstract: This study supports the continued use of third-generation cephalosporins, spectinomycin, and fluoroquinolone drugs for the primary treatment of gonococcal infections in Manaus. The occurrence of isolates with reduced susceptibility to azithromycin and ciprofloxacin underscores the importance of ongoing antimicrobial susceptibility monitoring to support decisions regarding appropriate drugs for the treatment of gonococcal infections.

“…Emerging resistance to extended-spectrum cephalosporins (19,29,30), as well as resistance to fluoroquinolone (15,41) and macrolide (8,10,13,23) antibiotics, among gonococcal isolates is an ongoing challenge for the treatment and control of gonorrhea infections. In this study, we have provided insight into the antimicrobial profiles of N. gonorrhoeae isolates collected over a large geographic area using past and present antimicrobial agents recommended for gonococcal treatment (5,6).…”

“…Despite strong regulatory rulings, most antibiotics have historically been available to anyone who requested them from unlicensed salespersons in private pharmacies; 37 such practices undoubtedly contributed to high levels of antimicrobial resistance in patients with communityacquired urinary infections 38 and gonorrhoea. 39 At the end of 2010, new legislation was passed to prohibit sales of antibiotics without a medical prescription, but whether it will be enforced eff ectively throughout the country remains to be seen.…”

“…These compounds are used extensively to treat community-acquired pneumonia and chlamydial infection (4,12). Although erythromycin and azithromycin, in the 1-g dose, are not recommended for treatment of gonococcal infection, azithromycin has been used in some parts of the world to treat gonorrhea (7,30). Increased gonococcal resistance to erythromycin has been noted since the 1960s (19), and resistance to azithromycin has been recently identified (7,13).…”
