Refining the assessment of pulmonary regurgitation in adults after tetralogy of Fallot repair: should we be measuring regurgitant fraction or regurgitant volume?

Abstract: PR(volume) and PR(fraction) are not interchangeable. PR(volume) may be a more accurate reflection of RV preload and may better represent physiologically significant PR as compared with PR(fraction).

“…In particular, the impact of pulmonary regurgitation (PR) on LVEF and RVEF remains ambiguous. 2,3,7,8 Accordingly, the aim of the study was to assess factors associated with LVEF and RVEF in a large cohort of patients after TOF repair.…”

“…Significant PR was defined as PRF of 20% or higher. 8 The presence of the RV outflow tract (RVOT) aneurysm or akinesia was assessed as described previously. 2 Data on inter-and intraobserver variability of the measurements of ventricular volumes and PR were published previously 10,12 and were comparable to those from other centers.…”

“…The optimal timing of pulmonary valve replacement depends not only on ventricular volumes but also ventricular functions, so the RV and LV systolic performance should also be taken into account [15]. Thirdly, the different assessing abilities of either PR volume or PRF was beyond the scope of the study and should be explored in future research [18]. Furthermore, the clinical experience of the RV/LV ratio is still limited.…”

“…A decade ago, echocardiographic measurements were widely used to measure the severity of PR. Now CMR is accepted as the "gold standard" technique to measure PR and assess RV volume [46][47][48].…”

“…Indications for PVR include a combination of PR and clinical symptoms or severe PR and moderate to severe RV enlargement [47][48][49]. It is presumed that PVR lowers RV volume that leads to decrease in QRS duration, which works as a preventable mechanism for VT and SCD [14,[50][51][52].…”
