Reflectance spectrophotometry in the gastrointestinal tract: Limitations and new applications

Abstract: We confirmed that reproducible measurement can be obtained by different investigators using standardized techniques. Standardization of endoscopic equipment is also necessary to overcome the significant limitation of endoscopic equipment on RS measurements. RS measurements can document mesenteric venoconstriction associated with systemic hypoxia and blood flow autoregulation associated with hemorrhagic hypotension.

“…They demonstrated coherence between LDF blood flow and oxygenation in the skin microcirculation . However, NIRS devices used in recent studies of tissue oxygenation have mostly been used to evaluate oxygen saturation at the capillary level . This can only reveal local information about oxygen utilization.…”

“…The system uses the principle of absorbance and scattering of light in tissues, not requiring pulsatile blood flow or tissue contact. In previous studies [17][18][19][20][21], visible light spectroscopy has been validated to be useful for the detection of tissue ischemia in human gastric and colonic mucosa.…”

“…For the human studies, we employed a video endoscope system (endoscope: Olympus CF 140 S, CF 130 S; light source: Olympus CLV‐U20 or CV‐100 Units; Olympus America Inc, Melville, NY, USA). We used the endoscopic technique for obtaining unbiased reproducible ISO 2 and IHB readings with low intra‐ and interobserver variability as described by our laboratory 22–29 . In short, the RS probe was passed through the working channel of the endoscope.…”

“…Sample size was determined by consideration of the mean and s.d. of IHB and ISO 2 measurements obtained in a previous study using video endoscopy 29 . We assumed a clinically significant difference between the saline and the acid treatment to be between 0.5 and 0.6 s.d.…”
