Regional Accelerated Phenomenon in the Mandible Following Mucoperiosteal Flap Surgery

Abstract: Striking remodeling activity occurs adjacent to the site of injury in orthopedic surgery. This reaction has been described as regional accelerated phenomenon (RAP), as it speeds up the healing stage. The phenomenon is a transient burst of localized remodeling process following surgical wounding of cortical bone. We explored whether RAP occurs following mucoperiosteal flap surgery in the jaw bone. Mucoperiosteal flaps were performed on 60 Wistar rats, either only on the buccal aspect or both on buccal and lingu… Show more

“…Furthermore, RAP is a transient phenomenon; therefore, patients must be evaluated every 2 weeks during treatment. 7,16 The effects of RAP can diminish with a decrease in tooth movement velocity over time, although the effects have been reported to last for approximately 4 months. 14,17 Elastomeric chains can be preferred for space closures.…”

“…Only mucoperiosteal flap reflection without any decortication, as studied by Yaffe et al, 18 could serve the RAP, resulting in widening of the periodontal ligament space and tooth mobility without any force application. However, the possible complications after flap surgery, such as crestal bone resorption and bone dehiscence were troublesome.…”

“…In a 1994 study by Yaffe et al, 79 the RAP was shown to be produced by elevation of a full-thickness mucoperiosteal flap in the mandible of rats. In the study, 60 Wistar rats were divided into three groups: a control group, a full-thickness mucoperiosteal buccal flap group, and a full-thickness mucoperiosteal buccal and lingual flap group.…”

“…In 1994, Yaffe et al 79 showed that the RAP could be induced by elevation of a full-thickness mucoperiosteal flap alone in rats. The rat model has also been used to show increases in transverse maxillary tooth movement after fiberotomy.…”
