Regression and progression of microalbuminuria in adolescents with childhood onset diabetes mellitus

Abstract: PurposeAlthough microalbuminuria is considered as an early marker of nephropathy in diabetic adults, available information in diabetic adolescents is limited. The aim of this study was to investigate prevalence and frequency of regression of microalbuminuria in type 1 (T1DM) and type 2 diabetes mellitus (T2DM) patients with childhood onset.MethodsOne hundred and nine adolescents (median, 18.9 years; interquartile range (IQR), 16.5-21.0 years) with T1DM and 18 T2DM adolescents (median, 17.9 years; IQR, 16.8-18.… Show more

“…4 Other longitudinal, observational studies have replicated these results in children and adults, in type 1 and type 2 diabetes, and in individuals from North America, Asia, and Europe. [5][6][7][8][9] Taken together, these studies provide strong evidence that microalbuminuria may not be the ideal marker of progression after all.…”

“…Urine albumin to creatinine ratio (UACR) in a spot sample is an early marker of kidney injury [13, 14]. Further, the presence of proteinuria has also been described as a risk factor for rapid progression and poorer prognosis of CKD [15, 16].…”

“…Microalbuminuria has been reported to occur in approximately 15% of children, especially those who demonstrate hyperfiltration. However, microalbuminuria has been shown to remit in a substantial portion of children and adults with other hyperfiltration disorders, notably diabetes mellitus . Therefore, the prognostic value of microalbuminuria in pediatric patients with SCD remains to be established.…”
