Regression of Ocular Adnexal Lymphoma AfterChlamydia Psittaci–Eradicating Antibiotic Therapy

Abstract: C psittaci-eradicating antibiotic therapy with doxycycline is followed by objective response in patients with ocular adnexal lymphoma, even after multiple relapses of the disease. A confirmatory, large, phase II trial is warranted to confirm whether this fast, cheap, and well-tolerated therapy could replace other more aggressive strategies as first-line treatment against ocular adnexal lymphomas.

“…Micro-organisms have been implicated in the pathogenesis of MALT lymphoma arising from some locations, 16 with successful treatment by using antibiotics, most notably doxycycline for orbital MALT lymphomas. [17][18][19] However, the results are still premature to consider it as a primary modality of therapy because durable remission has only been documented in 48% in a cohort of 27 patients treated 19 and that a uniformly high response rate has not been reported by other investigators. 20 However, it is a nonmorbid treatment, and therefore an attractive initial treatment approach and can spare or delay the use of cytotoxic therapy such as RT.…”

“…Subconjunctival intralesional injections of interferon-α were tried in a small group of patients, with promising results [119,132]. Recently, a pilot study of C. psittaci-eradicating therapy in patients with ocular MALT lymphoma was undertaken by the Italian group that initially published data on the association between the two entities [133]. Nine patients (five with relapsed or refractory disease) were treated with oral doxycycline (100 mg orally twice a day for 3 weeks).…”

“…Borrelia burgdorferi and Campylobacter jejuni are associated with cutaneous marginal zone B-cell lymphoma and immunoproliferative small intestinal disease, respectively (Cerroni et al, 1997;Lecuit et al, 2004). Recent studies show that Chlamydia psittaci is variably associated with ocular adnexal MALT lymphoma in different geographical regions (Ferreri et al, 2004;Chanudet et al, 2006) and eradication of the organism results in complete regression of the lymphoma in some cases (Ferreri et al, 2005).…”
