Objective: The purpose of this investigation was to determine whether an association exists between post-operative chemotherapy weight change and breast cancer control and survival, specifically within a high-risk population of women with node-positive breast cancer diagnosed and treated in the contemporary setting. Methods:A retrospective investigation with the following eligibility criteria: women who underwent primary surgical therapy for invasive breast cancer, were found to have pathologic evidence of nodal involvement, and were treated with adjuvant chemotherapy and radiation therapy. Demographic, tumor-, and treatment-related data were recorded and analyzed for association with disease control and survival outcomes. The post-operative/pre-chemotherapy weight and BMI served as the baseline comparator for future weight/BMI changes. Results:From January 2004 through December 2008, 52 patients were identified for inclusion. At a median follow-up of 77.2 months (range 19.9-119.4, with 85% followed >5 years), 8 patients experienced disease recurrence and 7 patients had died (3 with recurrent disease). Analysis of factors associated with study endpoints identified absolute weight and BMI change at 1, 3, and 4 years post-chemotherapy completion as significantly inversely associated with overall survival, but not associated with freedom from failure. Additionally, a statistically significant association between number of lymph nodes involved and freedom from failure was identified. Conclusion:This investigation did not detect statistically significant associations between weight or BMI change during chemotherapy and disease control or survival within this high-risk population, while the number of lymph nodes involved was directly related to risk of disease failure. Weight change at intervals following chemotherapy completion demonstrated some association with overall survival.

SUMMARYPurpose: The purpose of this study was to determine the frequency of body image and sexual problems in the first months after treatment among women diagnosed with breast cancer at age 50 or younger.Background: Breast cancer treatment may have severe effects on the bodies of younger women. Surgical treatment may be disfiguring, chemotherapy may cause abrupt menopause, and hormone replacement is not recommended.Methods: A multi-ethnic population-based sample of 549 women aged 22-50 who were married or in a stable unmarried relationship were interviewed within seven months of diagnosis with in situ, local, or regional breast cancer.Results: Body image and sexual problems were experienced by a substantial proportion of women in the early months after diagnosis. Half of the 546 women experienced two or more body image problems some of the time (33%), or at least one problem much of the time (17%). Among sexually active women, greater body image problems were associated with mastectomy and possible reconstruction, hair loss from chemotherapy, concern with weight gain or loss, poorer mental health, lower self-esteem, and partner's difficulty understanding one's feelings. Among the 360 sexually active women, half (52%) reported having a little problem in two or more areas of sexual functioning (24%), or a definite or serious problem in at least one area (28%). Greater sexual problems were associated with vaginal dryness, poorer mental health, being married, partner's difficulty understanding one's feelings, and more body image problems, and there were significant ethnic differences in reported severity.Conclusions: Difficulties related to sexuality and sexual functioning were common and occurred soon after surgical and adjuvant treatment. Addressing these problems is essential to improve the quality of life of young women with breast cancer.

ÖZETAmaç: Meme kanseri tarama programlarının uygulamadaki artışa bağlı olarak erken evrede tespit edilen meme kanseri oranı artmaktadır. Bu hastalara uygulanan cerrahi teknik tercihinde fikir birliği sağlanamamıştır. Çalışmamızda erken evrede tespit edilen meme kanserli hastalarımızın klinik özellikleri ile birlikte pratiğimizde uyguladığımız cerrahi teknikler incelendi Gereç ve Yöntemler: Süleyman Demirel Üniversitesi Tıp Fakültesi Hastanesinde 2013-2015 yılları arasında kliniğimizde erken evrede tanı konulan 95 hastanın dosyası retrospektif olarak incelendi. Bulgular: Kliniğimiz tanı ve tedavi edilen erken evre invaziv duktal karsinomlu hastaların tamamı kadındı. Hastalar ortalama 57 yaşında tanı aldılar. Hastaların %32,6'sına meme koruyucu cerrahi ve sentinel lenf nodu örneklemesi; %8,4'üne meme koruyucu cerrahi ve aksiller disseksiyon; %4,2'sine simple mastektomi ve sentinel lenf nodu örneklemesi; ve %54,7'sine de modifiye radikal mastektomi uygulandı. Erken evre meme kanserinde uygulanan cerrahi prosedürler içerisinde meme koruyucu cerrahi %41,4, sentinel lenf nodu örneklemesi %36,8 oranındaydı. Sonuç: Erken evrede tespit edilen meme kanserli hastalara uygulanan meme koruyucu cerrahinin veya mastektominin genel sağkalım ve hastalıksız sağkalım süreleri açısından birbirleri arasında fark izlenmemiştir. Kozmetik açıdan kuşkusuz daha başarılı olan meme koruyucu cerrahinin bu hasta grubunda ve klinik pratiğimizde artırılması fikrindeyiz. Results: All the patients diagnosed and treated in our clinic for early stage invasive ductal carcinoma were female. Patients were diagnosed at a median age of 57 years. The treatment applied was breast-conserving surgery and sentinel lymph node sampling in 32,6%, breast-conserving surgery and axillary dissection in 8,4%, simple mastectomy and sentinel lymph node sampling in 4,2% and modified radical mastectomy in 54,7%. Within the surgical procedures applied for early stage breast cancer, breast-conserving surgery was applied at the rate of 41,4% and sentinel lymph node sampling at 36,8%. Conclusion: No difference was determined between the patients with early stage breast cancer who underwent breast-conserving surgery and those who underwent mastectomy in terms of general survival and disease-free survival periods. From a cosmetic aspect, the breast-conserving surgery was without doubt more successful and is being increasingly used in our clinical practice.
