Rehabilitation of the severely atrophied dentoalveolar ridge in the aesthetic region with corticocancellous grafts from the iliac crest and dental implants

Abstract: BackgroundThe aim of this study was to assess changes in bone volume after block bone augmentation and placement of dental implants and further evaluate the aesthetic outcome of the treatment.Material and Methods9 Patients with atrophied anterior maxilla were included in this study. They received total of 21 implants. Dimensions of the alveolar ridge were measured from cone-beam computed tomography x-rays. The bone level at the implant sites was analysed from intraoral x-rays and the aesthetic outcome was asse… Show more

“…Moreover, the osteogenic capacity of rib bone grafts may be comparatively lower than that of iliac grafts due to a relatively lower proportion of cancellous bone and a corresponding scarcity of osteoinductive factors, including bone morphogenetic proteins (BMPs) and growth factors [ 42 44 ]. Conversely, the iliac crest, with its abundance of cancellous bone, serves as a robust source of osteoprogenitor cells and vascular marrow, facilitating superior osteoinduction and a vigorous osteogenic response after implantation [ 40 , 45 48 ]. Its denser cortical layer imparts enhanced resistance to compression, which is crucial for maintaining the stability of the anterior and middle columns post-surgery.…”

“…The aesthetic aspect of implants cannot be met without maintaining healthy soft tissue supported by a satisfactory height of alveolar bone level, 9 preventing the exposure of unwanted titanium fixture or its metallic shadow in the cervical region especially in the anterior region. 10…”
