Relation of Carotid Intima-Media Thickness and Plaque With Incident Cardiovascular Events in Women With Systemic Lupus Erythematosus

Abstract: Patients with systemic lupus erythematosus (SLE) are at increased risk for cardiovascular (CV) disease. The aim of this study was to investigate the association between subclinical CV disease as measured by carotid intima-media thickness (IMT) and plaque using B-mode carotid ultrasound and incident CV events in a combined cohort of female patients with SLE. This was a prospective, 2-center observational study of 392 adult women with SLE and no previous CV events with a mean 8 years of follow-up. Incident CV ev… Show more

“…There are also significant data in other diseases that share pathophysiological overlap with HIV. Specifically, the mechanisms of increased vascular risk in HIV are likely related to immune activation and an increase in inflammation 24, 25 and in other diseases where an immune activation and inflammation play a key pathophysiological role, an increase in incidental carotid plaque has been reported 26, 27 . For example, in rheumatoid arthritis and diabetes, data have described an increase in incidental carotid plaque 28, 29 .…”

“…In large studies without comparison with healthy controls, Manzi and colleagues ( n  = 175) [41] and Kao and colleagues ( n  = 392) [20] have observed carotid plaques in 30 to 40% of SLE patients.…”

“…In addition, these risk factors significantly contribute to intima-media thickness and plaque progression, which are reproducible markers of subclinical ATS and useful tools for prediction of CV events, as recently demonstrated in patients with systemic autoimmune diseases [2][3][4]. In this setting, it is now clear that chronic inflammatory/autoimmune rheumatic diseases are characterized by an acceleration Original Article of ATS.…”
