Relationship between atlanto-odontoid osteoarthritis and idiopathic suboccipital neck pain

Abstract: We discuss the relationship of atlanto-odontoid (AO) (anterior C1-C2 joint) osteoarthritis to suboccipital pain. A questionnaire regarding suboccipital neck pain was presented to 210 consecutive patients undergoing computed tomography (CT) of the brain or sinuses for a variety of indications. In all patients the AO joint and the lateral scout image of the cervical spine were studied. In 104 (49%) degenerative changes were seen at the AO joint. There were 89 patients (42%) who reported pain in the suboccipital … Show more

“…Little is known, however, about OA of the median atlanto-odontoid joint in living individuals. In agreement with Silberberg (1977), Skaane and Klott (1981), Zapletal et al (1996Zapletal et al ( , 1997, Prescher et al (1996), and Prescher (1998), we have found that significant widening and enlargement of the median articular planes of the anterior arch of the atlas and the dens axis lead to an extensive osseous overgrowth of the articular planes and the development of osteophytes of variable size and form. Prescher et al (1996), Prescher (1998), and Prescher and Brors (1998) designated large superior osteophytes of the atlas and on the tip of the dens axis coulisses, crowns, and peridental aureoles, in view of their shape.…”

“…If the usual pain is reproduced during the injection, it is a good sign that the procedure will be successful. 11,12 After skin asepsis, a sterile, autoadhesive, windowed draping is placed to cover the head and neck of the patient. A 20-gauge, 8-cm-long, spinal needle is used.…”
