Relationship between hypercoagulable state and erythrocyte phosphatidylserine exposure in splenectomized haemoglobin E/β‐thalassaemic patients*

Abstract: Summary. Small pulmonary arterial thromboses can occur following splenectomy of patients with haemoglobin E/bthalassaemia (Hb E/b-thal). We compared plasma markers of coagulation activation in vivo and red blood cell (RBC) markers of procoagulant activity in 15 Hb E/b-thal patients who were not splenectomized (NS), 15 who had been splenectomized (S), and 15 normal controls (NC). Levels of plasma thrombin-antithrombin III complex (TAT) were significantly higher in the S group than in either the NS or the NC gro… Show more

“…To ascertain the orientation of these extruding vesicles, cultured reticulocytes were stained for PS and with mAb to intracellular epitopes BRIC163 17 (GPA) and BRIC155 18 (AE1), and shown to be inside-out ( Figure 1D). To control for correct PS staining, we treated red cells with N-ethyl maleimide, followed by Ca 21 and ionomycin as Kuypers et al 13 (supplemental Figure 1). We have tested in vitro-produced reticulocytes from numerous different cultures and always observe that 5% to 10% of reticulocytes have an external PS-positive inside-out vesicle.…”

“…20 An increased number of PS-exposed red cells has also been observed by flow cytometry in circulating red cells after splenectomy. 21 We analyzed red cells from two splenectomized but otherwise healthy individuals. In both cases, splenectomy was performed because of immune-mediated thrombocytopenia and the patients' hematologic For personal use only.…”

“…12 Circulating MP can be very noxious in different clinical settings such as acute coronary syndromes, atherosclerosis, post-transfusional acute respiratory distress syndrome, diabetes mellitus, systemic inflammatory disease and disseminated intravascular coagulation. 10,12,36,39,40 MP originating from RBC membranes have also been considered a major cause of premature atherosclerosis described in TI patients. 13 The mechanisms responsible for the generation of MP appear very complex in different cell types 32 and very little is known about thalassemic RBC.…”

“…It is proven that thalassemic red blood cells have procoagulant activity [15]. Furthermore, a case study done on a splenectomised haemoglobin E/β thalassemia woman with pulmonary arterial hypertension showed normalization of thrombin-antithrombin level and reduce of pulmonary vascular resistance occur after chronic transfusion of packed red blood cells was given in addition to other ongoing treatment with warfarin, acetyl salicylic acid, desferrioxamine, and other supportive measures [16].…”
