Relationship Between Work Context and Adherence to a Clinical Practice Guideline for Peripheral Venous Catheters Among Registered Nurses in Pediatric Care

Abstract: Our findings indicate that components in one CPG might require diverse implementation strategies because they are linked to different contextual factors.

“…Guidelines are the only way for nurses to become familiar with all the relevant and best evidence . Clinical practice guidelines (CPGs) are useful tools based on the best available evidence for bridging the gap between available scientific evidence and clinical practice . Implementation of CPGs in practice has positive effect on health professionals’ practice and patient outcomes .…”

“…9 Clinical practice guidelines (CPGs) are useful tools based on the best available evidence for bridging the gap between available scientific evidence and clinical practice. 10,11 Implementation of CPGs in practice has positive effect on health professionals' practice and patient outcomes. 10 Despite increasing the number of CPGs, they have had a limited effect on changing professionals' , especially nurses' , behavior in patients care.…”

“…A cluster‐randomized controlled study on the effects of computer reminders on guideline implementation was conducted in a pediatric university hospital in Sweden; the Reminder Implementation for Safety in Child Care (RISCC) project. The project focused on registered nurses’ (RNs’) adherence to a CPG on peripheral venous catheters (PVCs; Förberg et al., ). PVCs as a healthcare device are provided: A PVC is a thin plastic catheter inserted into a vein for access to the blood stream. It is an important and sometimes vital device for treatment and survival; with PVC access, the patient can receive fluids, medication, nutrition or blood products intravenously. A PVC is associated with risks such as: leakage, occlusion, infiltration, phlebitis, or infection (Statens Beredning för Medicinsk och Social Utvärdering [SBU], , ). …”

“…RISCC has shown that while a majority (61%) of the RNs reported that they knew of the CPGs for venous catheters, adherence to its content varied greatly, at both individual and unit level (Förberg et al., ); fewer years since attaining an RN certificate, and regular feedback on performance were found to correlate with a tendency to self‐report adherence to disinfection of hands and use of gloves while managing a PVC. While quantitative studies and self‐reports can shed certain light on effects, in‐depth understanding may benefit from supplementary data collection and analysis (Creswell & Plano Clark, ).…”
