Relevance of delayed hospital admission on development of cardiac rupture during acute myocardial infarction: study in 225 patients with free wall, septal or papillary muscle rupture

Abstract: Delayed hospital admission or undue in-hospital physical activity appears to increase the risk of rupture in patients prone to this complication (i.e., a first transmural AMI, absence of overt heart failure and advanced age); recurrence of ischemia/infarction emerges as a potential additional trigger in a proportion of these patients.

“…12 Risk factors include age over 50 years and first MI; hypertension and diabetes have not been found to be risk factors in a recent large study. 14 Delay in hospital admission of more than 24 hours after MI is significantly associated with post-infarction cardiac rupture 15 ; other literature suggests that cardiac rupture is the presenting complaint in a subset of patients with asymptomatic MIs. 16,17 Up to one third of ruptures occur in a delayed fashion, three to six days after MI.…”

“…This patient had many of these risk factors. 8,9 When there is suspicion of a mechanical complication after myocardial infarction, an echocardiogram obtained immediately, as in this case, can narrow the differential diagnosis and increase the speed to the correct diagnosis and treatment. 10 Most of the conditions in Table 2 have unique features on ultrasonography.…”
