Reliability of morning, before‐dinner, and at‐bedtime home blood pressure measurements in patients with hypertension

Abstract: The authors evaluated differences in the reliability of home blood pressure measurements taken in the morning, before dinner, and at bedtime. Forty-eight patients with hypertension (age range, 50-89 years; mean age, 76.4 years) measured their home blood pressure using a validated automatic information/communication technology-based device for 14 consecutive days. Those days were divided into the first seven days (1-7) and the following 8 to 14 days (days 8-14) and compared systolic blood pressure (SBP) reliabi… Show more

“…[47][48][49][50] Lifestyle factors in Asia mean that evening BP is difficult to measure before dinner at home, and evening home BP measured at bedtime after dinner is significantly affected by alcohol consumption and bathing, 51 making this approach much less reliable, as has been shown previously. 52 In addition, as highlighted above, there is a good body of data from…”

“…Lifestyle factors in Asia make it difficult to measure evening BP before dinner; therefore, the Japanese Society of Hypertension guidelines recommend that evening home BP is measured just before going to bed . However, measurements at this time are strongly affected by dinner (including alcohol consumption) and evening behavior (eg, bathing in the evening, which is common in Asia) . Therefore, the reproducibility of evening home BP measured just before going to bed is worse than that for morning home BP .…”

“…Home BP monitoring (HBPM) may be the best approach for detecting seasonal variation in BP. By using HBPM, one can detect BP levels and variability according to various time phases, such as morning‐evening, day‐by‐day, and seasonal variations . Morning home BP is more closely associated with the indoor room temperature than with the outdoor temperature, and the average home BP monitoring, especially in the morning, is significantly associated with cardiovascular risk in community‐dwelling individuals and hypertensive patients .…”
