Reliability of the European Society of Human Reproduction and Embryology/European Society for Gynaecological Endoscopy and American Society for Reproductive Medicine classification systems for congenital uterine anomalies detected using three-dimensional ultrasonography

Abstract: The low reliability of the ESHRE-ESGE system may lead to a lack of consensus about the management of common uterine malformations and biased research interpretations. The use of the ASRM classification, supplemented with simple morphometric criteria, may be preferred if their sufficient reliability can be confirmed real-time in a large sample size.

“…The reliability of currently used measurements may be compared with four available studies 11,17,33,34 . Revised interpretation of these studies' results using newly proposed cut-offs confirms that the reliability of measurements of internal indentation depth is significantly better than those of the indentation angle and the uterine wall thickness 23 .…”

“…Instead they adopted a definition of uterine septum, if there is an IILFM > 50% of myometrial wall thickness [12]. Although ESHRE-ESGE classification has been criticized in few reports, it provides the long overdue morphometric criteria needed to make a diagnosis [16,20,21]. The main criticism is over diagnosis that may lead to over treatment.…”

“…While this most recent classification system has overcome some problems associated with previous systems [41], it was shown in a comparison of the ESHRE-ESGE classification with the ASRM classification using 3D US [42] that the application of the ESHRE-ESGE criteria resulted in a relative overdiagnosis of septate uteri and insufficient inter-and intrarater reliability [43], which potentially might lead to unnecessary surgical overtreatment.…”
