Renal involvement in patients with polymyositis and dermatomyositis

Abstract: Renal involvement in patients with polymyositis (PM)/dermatomyositis (DM) is previously thought to be uncommon, but two main types of renal lesion have been described. First, acute tubular necrosis with renal failure related to myoglobulinemia and myoglobulinuria is a well-recognised feature of acute rhabdomyolysis. Second, chronic glomerulonephritis has been infrequently reported in a small group of patients with PM/DM. This study aims at investigating the incidence, severity and prognosis of renal disease in… Show more

“…However, according to a recent report of 14 PM/DM patients [1], its incidence rate is 25.1% (14 patients). These patients are categorized into myoglobulin-related acute tubular injury (ATI) and glomerulonephropathy.…”

“…Therefore, if only we could precisely diagnose every ATI as renal involvement in PM/DM, the incidence rate will not be low. In the study by Yen et al [1], 9 of the 14 PM/DM patients (64.3%) with renal involvement had ATI but only 2 patients (14.3%) had biopsy-proved glomerulonephropathy. The incident of glomerulonephropathy is still rare.…”

“…Mesangial proliferative glomerulonephritis is observed most frequently in idiopathic PM. [9] Yen et al [10] reported 14 cases of renal involvement in 65 PM/DM patients. In five polymyositis patients, four developed acute tubular necrosis with renal failure related to myoglobulinuria and myoglobulinemia following acute rhabdomyolysis, and emergent hemodialysis was necessary in one of them.…”
