Renal Vulnerability to Drug Toxicity

Abstract: Drug-induced kidney disease occurs primarily in patients with underlying risk factors. A number of factors enhance the vulnerability of the kidney to the nephrotoxic effects of drugs and toxins. They are broadly categorized as patient-specific, kidney-related, and drug-related factors. One, two, or all three of the factor categories can act to promote various forms of renal injury. Importantly, all compartments of the kidney can be affected and result in one or more classic clinical renal syndromes. These incl… Show more

“…The main feature of the AKI is the reduction of glomerular filtration rate, which results in accumulation of nitrogenous products -urea and serum creatinine -a medical condition called azotemia (16) . Patients with minimal reduction in filtration rate have risk factors for AKI (7) .…”

“…Toxic AKI involves numerous intrinsic factors, some nonmodifiable, such as high age, gender, hypertension, chronic renal failure, congestive heart failure, multiple myeloma and modifiable factors such as dehydration, medication dosage, hypotension, concomitant use of nephrotoxic drugs, shock and sepsis (7,14,21) .…”

“…(6) Examples include antibiotics, chemotherapeutic agents, analgesics and immunosuppressants. (7) The cyclophosphamide stands out among the chemotherapeutic drugs. It is a cytostatic agent used in the treatment of neoplastic diseases of solid tumors, lymphomas and other non-neoplastic diseases, such as rheumatoid arthritis and systemic lupus erythematosus.…”

“…Briefly, the cause of kidney injuries could be classified into categories based on: a) toxicity by drugs and toxins, which leads into a kidney injury state (22), and b) genetic basis, which involves the genes governing the structural features of the glomerular basement membrane (23). Indeed, the structural organization of slit diaphragm and the podocyte foot processes constitute the basis of glomerular selectivity (11)(12), and any change in these features would lead to kidney injury like condition with excessive proteinuria.…”
