Rendezvous endovascular common carotid artery stenting (RECCAS) technique for symptomatic steno-occlusive disease

Abstract: This report describes a patient who presented with acute but transient right arm weakness and altered sensation secondary to severe stenosis of the left common carotid artery (CCA) origin. Endovascular stenting of the stenosed origin was achieved utilising a novel rendezvous technique through combined retrograde common carotid artery and anterograde transfemoral approaches. This technique has numerous potential advantages over traditional transfemoral endovascular and open retrograde common carotid artery appr… Show more

“…An antegrade approach with some techniques has been reported, such as the buddy wire technique, using the gooseneck snare, distal balloon anchoring, stenting after the lesion crosses the guiding catheter, sheath pull-through technique, and the pull-through technique with direct puncture of the CCA. [6][7][8][9][10][11] These techniques mainly focused on the stability of the guiding catheter. In addition, there are some risks of perioperative stroke.…”

“…Retrograde CCA stenting (RCCAS) is a very well-described procedure that is often performed for stenosis or occlusion of the innominate artery or the CCA [ 11 ]. This may be accomplished as an isolated endovascular procedure or in conjunction with CEA to achieve the patency of the CCA [ 12 , 13 ]. This technique may also satisfy the recommendations provided by the ESVS advocating for an endovascular approach as a first option.…”
