Background:To compare results of tricuspid valve (TV) reoperation between incision via a right anterolateral minithoracotomy (RAMT) and median sternotomy (MS). Methods:We performed a retrospective analysis of 49 patients who underwent isolated TV surgery as a reoperation at our institution between 2006 and 2015. Previous cardiac operations included mitral, aortic, and TV surgeries, atrial septal defect repair, and pericardiectomy. The mean age of the patients was 51.9±12.8 years, 14 (28%) were male and 35 (72%) were female. Follow-up was 95% (38/40) complete, with a mean duration of 41.3±19.5 months.Results: Perioperative demographic and laboratory tests did not show any significant differences between the RAMT and MS groups. The drainage volume, total red cell unit, total serum volume and platelet were significantly different 1150±803.5/2,270±1,920, 4.8±4.1/8.7±8.9, 478.2±488.9/950.0±857.6, 0.04±0.21/0.38±0.64 (P<0.05), while other perioperative data were similar. There were no significant differences in early postoperative death and complications between the RAMT and MS groups.A multivariate linear regression analysis predicted that serum creatinine (Scr), age, and MS group were independent risk factors for bleeding. The Cox regression demonstrated that the MS group had a longer drainage duration (P<0.05) and had a relative hazardous risk (HR) of 2.691 (1.328, 5.450 CI) compared with the RAMT group. Conclusions:The RAMT approach is an alternative, safe, and feasible procedure for isolated TV reoperation. It has the advantages of less drainage and reduced requirement for blood products.

Artykuł specjalny/Special articlePierwotne wady zastawki trójdzielnej -zarówno wrodzone, np. anomalia Ebsteina lub zastawka dysplastyczna, jak i nabyte, tj. reumatyczne, pozapalne lub w przebiegu rakowiaka -występują rzadko. Najczęściej spotyka się niedomykalność trójdzielną w schyłkowym okresie niewydolności serca, zwykle w przebiegu choroby zastawkowej lewego serca. Ostatnio, wraz z coraz większym upowszechnieniem nowoczesnych metod leczenia za pomocą stymulatorów, kardiowerterów-defibrylatorów i stymulatorów resynchronizujących (ang. cardiac resynchronization therapy, CRT), coraz częściej spotyka się chorych z jatrogenną niedomykalnością trój-dzielną spowodowaną obecnością elektrody w prawej komorze.Operacyjne leczenie wady zastawki trójdzielnej wiąże się z dużym ryzykiem i zwykle jest wykonywane przy okazji innego zabiegu kardiochirurgicznego, u chorych w cięż-kim stanie, ze schyłkową niewydolnością serca lub cięż-ką wadą zastawkową lewego serca. Wyniki leczenia operacyjnego nie są dobre, a reoperacje wiążą się z bardzo dużym ryzykiem. Śmiertelność szpitalna po reoperacji z powodu niedomykalności trójdzielnej sięga 37% [1][2][3]. Ze względu na duży odsetek powikłań zakrzepowych związanych z zastawkami mechanicznymi obecnie w ujście trójdzielne wszczepia się głównie zastawki biologiczne, które po kilku, a najpóźniej kilkunastu latach ulegają degeneracji, co powoduje konieczność wykonania reimplantacji. Problem ten dotyczy szczególnie młodych pacjentów po operacji wrodzonych wad zastawki trój-dzielnej, którzy w swoim życiu mogą wymagać kilku wymian zastawki biologicznej. Pierwsze interwencje przeznaczyniowe w wadzie trójdzielnej lub dysfunkcji bioprotezy trójdzielnejOd 2000 roku, kiedy Boenhoffer wykonał pierwszy zabieg przeznaczyniowego wszczepienia zastawki płucnej, metoda ta stała się procedurą powszechnie stosowaną u chorych dużego ryzyka z wadą aortalną oraz coraz czę-ściej wykorzystywaną u pacjentów z wadami wrodzonymi po operacjach rekonstrukcji drogi odpływu prawej komory [3][4][5][6][7][8][9]. Ostatnio pojawiły się pierwsze doniesienia o przeznaczyniowym -ortotopowym lub heterotopowymwszczepieniu zastawki trójdzielnej.Pierwszy zabieg przeznaczyniowej implantacji zastawki w pozycji trójdzielnej wykonano u 31-letniego pacjenta z atrezją trójdzielną po operacji Fontana w modyfikacji Björ-ka, u którego w wyniku zwapnienia homograftu doszło do wystąpienia ciężkiej niedomykalności zastawki homograftu. Zabieg wykonano w znieczuleniu ogólnym z dostępu przez żyłę udową pod kontrolą echokardiografii przezprzełykowej. Wszczepienie zastawki Melody™ poprzedzono implantacją metalowego stentu. Pacjent opuścił szpital w dobrym stanie następnego dnia [7].Od tamtej pory opisano kilkanaście przypadków przeznaczyniowego wszczepienia zastawki Melody™ w pozycję trójdzielną (ortotopowo, tj. w miejscu zastawki, lub heterotopowo -pozasercowo) u chorych z wadami wrodzonymi, u których wcześniej wykonano operację wszczepienia biologicznej zastawki trójdzielnej lub konduitu łączą-cego prawy przedsionek z prawą komorą (operacja Fo...

Functional or secondary tricuspid regurgitation (TR) has seen increased attention in recent times as relationships with clinically-relevant outcomes have come to light. Despite the association of increased mortality with significant TR, the disease remains under-recognized and thus relatively untreated. In addition, the disease itself has not been extensively studied and the interactions between annular dilatation, right heart disease and pulmonary hypertension are poorly understood. However, the high mortality and recurrence rate with current surgical replacement or repair techniques is well recognised, opening the door to transcatheter therapies for functional TR. The current perspective reviews the rationale for transcatheter solutions, describes some of the current approaches and discusses the ongoing questions of a poorly-studied condition which may limit the design of clinical trials for this disease.
