“…In addition, several reports have also mentioned that DFI after the first hepatectomy is an important prognostic factor (9,10,12,19). Patients with a longer DFI after the initial hepatectomy and with negative lymph nodes at the primary site may be therefore good candidates for repeat reduction surgery.…”

“…In this study, we first tried to identify the profile of patients with a long-term survival after recurrence and who had previously undergone an initial hepatectomy. There have been several reports describing the survival for repeat liver (7)(8)(9)(10)(11)(12)(13) and pulmonary (14)(15)(16)(17) resection in patients with recurrence after a curative resection of colorectal metastases. On the other hand, there have also been few reports about such treatments for extrahepatic and extra-pulmonary metastases after the initial hepatectomy (18,19).…”

“…Most factors were related to the initial hepatectomy and included the following: a low serum level of [17,23,26,41] carcinoembryonic antigen (CEA) (variable threshold: less than 50 ng/mL or less than 5 ng/mL), the presence of small (less or equal to 4 cm) liver metastases, initial R0 resection, metachronous metastases, a long interval between the initial and subsequent hepatectomy (more than 6 months or more than 12 months). Other factors were related to the repeat hepatectomy including: the absence of extrahepatic disease at the time of repeat hepatectomy, R0 re-resection, low serum CEA, presence of a single metastasis, and small (less or equal to 5 cm) size of metastases.…”

“…Survival after repeated resections is as good after the first procedure. In two recently published Japanese series, the percentage of treatable patients with a second resection after the development of an intrahepatic recurrence ranged from 14.3 to 25.4%, and the 5-year survival was 49% [73,74].…”
