Repigmentation and curling of hair after acitretin therapy

Abstract: Drug-induced hair colour and texture changes have been reported rarely. We describe a 70-year-old woman with psoriasis who noticed darkening of her previously white hair, which also gained a curly appearance after 6 months of acitretin treatment. We present this case to emphasize that acitretin can be added to the list of drugs that induce changes in hair colour and texture.

“…There is no original work in the available literature which has reported the effects of isotretinoin on hair. Individual studies indicate that acitretin often causes hair loss [38, 39] and Seckin and Yildiz [40] reported a change in the color and structure of the hair. A decrease in the proportion of anagen hair observed in our patients can appeal for telogen effluvium hair loss.…”

“…[72,73] Darkening of hair has also been reported as an incidental finding with other drugs such as defibrotide, cyclosporine, corticosteroid, etretinate, L-thyroxine, verapamil, tamoxifen, levodopa, cisplatin, acitretin, tri-iodothyronine, and lenalidomide. [71,[74][75][76][77] However, in most of these cases, drug-induced etiology of pigmentation could only be confirmed if hair had returned to its original color after drug withdrawal. The same could not be confirmed as most patients were continued on treatment with the suspected drug.…”

“…Kinking of the hair (in addition to pigmentation changes) has also been reported with the use of acitretin and etretinate, which are thought to be associated with a change in keratinization patterns within the inner root sheath. 17,18 To our knowledge, this is the first report of a patient with AA who experienced a change in hair structure that recapitulated the patient's hair texture as a child. The precipitating factor for this change may have been a result of treatment of AA, pathophysiologic changes associated with the AA, or a combination of both.…”
