Replacement Lipomatosis of the Kidney: Mri Features

Abstract: Renal replacement lipomatosis of the kidney is characterized by renal sinus and perirenal fat proliferation. It is associated with chronic infection and calculi, commonly central, often obstructing. The kidney may be large or small but is usually nonfunctioning. Most of the renal parenchyma has been replaced by fat, pararenal fascia are thickened, and there may be fistulae. We reported radiological findings of renal replacement lipomatosis in a 55 year-old man.

“…It generally occurs unilaterally, contrary to renal sinus lipomatosis, and extends to the perirenal and periureteral areas. 3,4 Calculi are also seen in approximately 70% of cases. 3 Xanthogranulomatous pyelonephritis (XGP), malakoplakia and tumours including fat-like lipomas, …”

“…1 While renal sinus lipomatosis is a mild proliferation of fatty tissue in the renal sinus, RRL is considered to be an advanced or aggressive form of renal sinus lipomatosis. 2,3 The condition is the result of severe atrophy of the renal parenchyma and fatty proliferation and is usually accompanied by chronic infections, obstructive kidney stones and long-standing hydronephrosis. It generally occurs unilaterally, contrary to renal sinus lipomatosis, and extends to the perirenal and periureteral areas.…”

“…Normally the renal sinus contains fat tissue but in the parenchymal destruction or atrophy, endogenous or exogenous steroid exposure and obesity; the amount of fat increases. Although there can be a mass effect of the renal sinus lipomatous' on the collecting system it is not always symptomatic and generally detected bilaterally [1,2]. Renal replacement lipomatosis (RRL) is a milder version of renal sinus lipomatosis that generally detected unilaterally; associated with aging, renal atrophy, chronic urinary tract infections and more often related to the chronic calculous disease [1,3].…”
