Replication study of ADOS‐2 Toddler Module cut‐off scores for autism spectrum disorder classification

Abstract: The Autism Diagnostic Observation Schedule, Second Edition (ADOS‐2) has been regarded as the gold standard assessment of autism spectrum disorder (ASD). While clinical validity of ADOS‐2 Modules 1–4 have been extensively studied, there has been very limited research examining the clinical validity of ADOS‐2 Toddler Module. The goal of this study was to examine alignment of the ADOS‐2 Toddler Module classification with clinicians' determination of ASD, with assessing cut‐off scores for diagnosing ASD in toddler… Show more

“… 17 There have been multiple studies demonstrating the clinical utility and accuracy of the ADOS-2 across national and international samples. 20 , 21 , 22 , 23 , 24 , 25 , 26 , 27 , 28 However, to our knowledge, there have only been 2 studies examining ADOS measurement bias at the item level, using item response theory (IRT), by sex and/or race. Specifically, Harrison et al 16 investigated the role of race, ethnicity, and sex on 10 items of the ADOS-Generic.…”

“…Our findings show that TDAS ≥20 points yielded high accuracy (82.05%), sensitivity (82.86%), and specificity (80.39%) for ASD diagnosis. Compared to a recent study to find the optimal cut-off point for ADOS-2 in toddlers aged 12–30 months old in the US, the cut-off points were 12 and 10 based on Algorithms 1 and 2, respectively (Algorithm 1 for toddlers aged 12–20 months old or 21–30 months with <5 words and Algorithm 2 for toddlers aged 21–30 months old with ≥5 words) [ 30 ]. The sensitivity values were 98.31% and 95.65%, the specificity values were 75.41% and 84.78%, and the accuracy values were 93.60 and 91.30 for Algorithms 1 and 2, respectively.…”

“…Other than the TDAS score, our findings show that the age of the children was independently associated with ASD. As shown in a recent study in the US, cut-off points for toddlers were different between each age group [ 30 ]. However, the researchers did not attempt to find the optimal cut-off point for the subgroups due to the limited number of children in each age range.…”

“…The ADOS-2 has become more internationally accessible, driven by increased ASD awareness as well as the efforts to administer the ADOS-2 in different countries [ 13 ]. Currently, the ADOS-2 has been translated into more than 20 languages [ 14 ], and the clinical validity of the ADOS-2 has been well-established in various international samples [ 15 ]. Previous studies have underscored that ASD diagnostic instruments developed in Western countries can be properly translated and adapted in non-Western countries [ 13 , 16 18 ].…”
