Abstract: While this commentary is critical of our article, we very much welcome debate and conversation of scientific work; after all, it is through allowing such debates and exchange of ideas, that scientific knowledge grows. Moreover, the letter by Barrett et al 1 demonstrates the importance of pursuing population-based, epidemiological investigation using large-scale routinely collected data as a way to inspire further inquiry and research; that further inquiry was, in this instance, a detailed case study.
