28 female uraemic patients treated by chronic haemodialysis were studied. Of these patients, 6 had regular cycles, 6 irregular cycles, 7 were amenorrhoeic and 9 post-menopausal. Plasma gonadotropins (FSH and LH) were determined in all patients. Plasma concentrations of oestradiol and progesterone were determined only when of possible interest. LRH stimulation test was performed in 5 amenorrhoeic patients. In 19 pre-menopausal women baseline plasma FSH values were always normal, but LH concentrations were above normal in 5. In all 9 post-menopausal women both FSH and LH were markedly higher than in the pre-menopausal patients, either healthy or uraemic. Persistent oestrogen activity was found in the 6 regularly menstruating women as well as in the 5 women with oligomenorrhoea. Luteal phase plasma progesterone was in the low normal range in 3 patients and was markedly depressed in the others. A prolonged rise in LH and to a lesser extent in FSH was observed in 4 of the 5 patients studied after 100 µg of LRH were administered intravenously. These results suggest that the defect underlying gonadal dysfunction in uraemic women treated by chronic haemodialysis is mainly of suprahypophyseal origin.

A B S T R A C T The metabolic clearance rate (MCR) of human luteinizing hormone (hLH) has been determined in 10 normal men, 3 normal women, and in 12 women with ovulatory disorders resulting in oligomenorrhea or amenorrhea. The MCR was determined by the constant infusion technique using either iodinated or unlabeled highly purified hLH, and these results were compared to MCR determined by using crude pituitary preparations containing both follicle-stimulating hormone and hLH. Both preparations produced essentially similar results for the MCR of hLH and virtually identical results were obtained when complete or incomplete immunoprecipitation of the infused material was achieved. The MCR/body surface area of hLH was significantly greater in normal men (25.6+3.6 ml/minm2) than in normal premenopausal (19.2±0.9 ml/min. in2) or postmenopausal women (17.4±1.9 ml/min mn). No difference was noted in the MCR of hLH in women with oligomenorrhea or amenorrhea. Production rates (PRs) were calculated by using a pituitary standard, the values being 85.1±21.5 IU/24 h in normal men, 39.9±12.6 IU/24 h in normal premenopausal women, and 294.6±61.9 IU/24 h in normal postmenopausal women. The initial half-times of disappearance of the a-and P-subunits of hLH were measured in two normal men and found to be 15-18 min, respectively. The half-time of intact hLH was twice as great.

A B S T R A C T The effect of sex hormones on bone tissue was studied in 12 osteoporotic patients. Surfaces of bone undergoing formation and resorption were determined by quantitative microradiography of iliac crest biopsy samples before and after treatment with estrogens in 11 postmenopausal women and with testosterone in one gonadally competent man. Before treatment, bone resorption was greater than normal in all but one patient and bone formation was normal. After treatment, bone resorption decreased to within the normal range in all patients, and bone formation did not change significantly. Biochemical studies showed significant decreases in serum calcium, phosphorus, and alkaline phosphatase levels and in urinary excretion of calcium and hydroxyproline. These changes are believed to be the consequence of the effect of the hormones on bone. The data indicate that the major effect of sex hormones in osteoporosis is an inhibition of bone resorption. INTRODUCTIONAlbright, Smith, and Richardson in 1941 proposed that anabolic sex hormones were necessary for bone matrix synthesis, and that the loss of estrogens at the menopause in women or of androgens with aging in men caused osteoporosis (1). Since then, sex hormones have been used extensively in the treatment of osteoporosis, although their mechanism of action and therapeutic usefulness have not been completely established.Estrogens stimulate intramedullary bone formation in birds (2), but their effect on mammalian bone is variable.
