Objectives. The purpose of this study was to assess intraobserver and interobserver reproducibility of the parameters of ovarian response and oocyte ability, studied by 3-dimensional ultrasonography and power Doppler angiography (PDA), and the possible influence of the ovarian functional stage. Methods. Twenty-nine women were included in an in vitro fertilization program. Fourteen women were evaluated after pituitary suppression (basal group), and 15 were scanned on the human chorionic gonadotropin administration day, after gonadotropin ovarian stimulation (stimulated group). A first observer acquired 2 volumes for each ovary. Another observer performed a second analysis of the volumes acquired by the first observer. We analyzed ovarian volume, follicle number in the basal group, vascularization index, flow index, and vascularization-flow index. The volumes were processed by the Virtual Organ Computer-Aided Analysis imaging program using plane A and 15°rotational steps. Results. Ovarian volume showed excellent intraobserver and interobserver agreement, with an intraclass correlation coefficient (Intra-CC) and an interclass correlation coefficient (Inter-CC) close to the unit. The Intra-CC and Inter-CC about the number of follicles were 0.964 and 0.978, respectively. Vascularity indices showed an Intra-CC greater than 0.90. The vascularization index and the vascularization-flow index showed higher interobserver than intraobserver reproducibility (Inter-CC > 0.96 for both). The flow index Inter-CC was 0.898. The reproducibility differences between the basal and stimulated ovary measurements were not significant. Conclusions. There is an excellent intraobserver and interobserver reproducibility of the ovarian volume, follicle counts, and 3-dimensional PDA indices. The ovarian functional stage has no influence on the reliability. Three-dimensional ultrasonography and PDA improve the study of ovarian parameters, and their reliability impels a change in the current clinical routine of performing and interpreting ultrasonography.

Background: Our aim was to describe the corpus luteum morphology by two-dimensional ultrasound correlated by its vascularization and volume by 3D ultrasound and study the possible relationship between serum progesterone levels and the corpus luteum morphology.Methods: Thirty-eight women were included in an intrauterine insemination program (IUI) in Santa Cristina University's Hospital. All the patients were evaluated in mesoluteal phase, the day +7 after hCG administration, by two-dimensional and three-dimensional ultrasound. The volume and vascular indices of the corpus luteum were calculated off-line using virtual organ computer-aided analysis (VOCAL TM ) software.Results: Four different morphologies were described in the corpus luteum: echo-positive, echo-negative or sonoluscent, mixed echogenicity or nonvisible. Corpus luteum with mixed echogenicity was the most frequent one with 37.5% (12 cases). The corpus luteum vascular indices change in each morphology type, but there was statistically significant association just in vascularization index between echo-negative and mixed echogenicity corpus luteum morphologies, with p = 0.034. The rest of vascular indices do not change in each morphology corpus luteum types. There was statistically significant difference in mean gray value between echo-negative and mixed echogenicity morphologies, with p = 0.007. There were no statistically significant correlations between the corpus luteum morphology and the corpus luteum volume of any of the different types. There either was no statistically significant correlation between the corpus luteum morphology and progesterone serum levels on day +7 postovulation. Conclusions:The mixed echogenicity corpus luteum morphology has more vessels and more cell mass than echo-negative ones. Progesterone serum levels in mid-luteal phase has no influence in corpus luteum morphology and vascularization.

Objectives: We used three-dimensional ultrasound with trans-abdominal Power-Doppler (3D-US PD) to determine placental vascular evolution in normal pregnancies (NP) and in insulin-dependent Gestational Diabetes Pregnancies (GDP). Study design:We obtained 473 measurements from 43 NP at 20 weeks-40 weeks gestation and 122 measurements from 70 insulin-dependent GDP at 22 weeks-40 weeks. Standardization was achieved recording three successive placental vascular tree volumes, measuring the spherical volume between the chorionic and basal plates always under or near funicular insertion. Parameters analysed were: mean intensity of blood flow (Flow Index, FI); percentage of volume occupied by vessels (Vascularisation Index, VI); and intensity of blood flow in the volume occupied by vessels (Vascularisation-flow index, VFI). Results:In NP, FI increased throughout early pregnancy and decreased before delivery and correlated with fetal parameters. In contrast, in GDP, FI was high from the onset, better correlated with placental parameters, and had a diagnostic cutoff value of 45, which was only found in 7% of NP at the end of the pregnancy. VI was variable and had a low diagnostic value, being related to placental parameters; VI values were significantly lower in NP (17.4 ± 7.4%) than in GDP (21 ± 12%) with a diagnostic cutoff point at 31%. In NP, FI-peak was at 32 weeks, two weeks after the VI-peak, while VFI showed no significant differences. Conclusions:The results showed that placental blood flow (FI) was related to fetal circulation, while the percentage of vessels per volume (VI) was related to maternal circulation. 3D-US PD indicated a diagnosis of GDP for FI > 45 and VI > 30%. We also discuss the values of change in VI that predict changes in fetal FI.
