Rescue transventricular off-pump mitral valve repair with artificial neochords for acute mitral regurgitation due to postinfarction papillary muscle rupture

Abstract: Background We report 3 cases of rescue transventricular off-pump mitral valve (MV) repair in high-risk patients with acute mitral regurgitation (MR) due to post–myocardial infarction (MI) papillary muscle rupture (PMR). Methods The 3 patients presented with acute inferior ST elevation myocardial infarction, cardiogenic shock, and pulmonary edema. Their preoperative peak troponin I levels were 1909 ng/L, 16,963 ng/L, and 8299 ng/L. All 3 patients underwent successful per… Show more

“…A new article has been published by Budra et al reporting successful treatment with rescue transventricular off-pump mitral valve repair with artificial neochords in three patients diagnosed to have post-MI PMR [13]. This case series is limited by its retrospective descriptive design and small sample size, as are other reports on PMR in the literature.…”

“…The posterior leaflet is captured and the neochords deployed and secured at the puncture site over Teflon strips (39). In the case series described by Budra and colleagues, three patients developed PMR after presenting with inferior STEMI, resulting in severe MR and cardiogenic shock (40). All three patients underwent off pump neo-chord implantation ranging from two to three chords with a chord also used to secure the sub-valvular apparatus.…”
