Resecting the great saphenous stump with endothelial inversion decreases neither neovascularization nor thigh varicosity recurrence

Abstract: Complete resection of the great saphenous vein stump and inversion suturing of the common femoral vein venotomy, instead of simple flush ligation at the level of the saphenofemoral junction, do not appear to decrease grade 2 neovascularization and related thigh varicosity recurrence after great saphenous vein stripping for primary varicose veins.

“…Als wahrscheinlichste Ursache gilt die Neoangiogenese. Doch sowohl der Einsatz diverser Barrier-Verfahren zur Verhinderung der postoperativen Krossenrezidive als auch neue endoluminale Therapieverfahren konnten das Auftreten einer Rezidivvarikosis nicht signifikant reduzieren (2,7,8,16,19,27,28,31,33).…”

“…47 Complete resection of the GSV stump 48 and inversion suturing of the common femoral vein did not seem to decrease neovascularization. 49 Results on the use of barrier materials are conflicting and these techniques have also not yet been introduced into common clinical practice. 50,51 Re-do surgery 52 Surgery is considered the best option when a major reflux may be identified due to a large stump.…”

“…1,2 Although arterial and capillary growth is the subject of many studies, the influence of different surgical vein occlusion methods on the inflammatory process and restoration of venous blood flow in tiny vessels is not evaluated yet. 3,4 The methods of vein stump closure have changed during the past decades; nowadays, the surgeon can choose among several techniques. The most commonly used methods are ligation using resorbable threads, use of titanium clips, or bipolar electrothermical coagulation.…”
