Resistance to Subsidence of an Uncemented Femoral Stem After Cerclage Wiring of a Fissure

Abstract: The load to initiate subsidence is increased in femora that have fissured, then have been stabilized with double-loop cerclage, when compared with intact femora. The relative subsidence is not different between intact and stabilized specimens.

“…In 4 dogs where it was perceived that there was a risk of creating a femoral fissure during broaching for femoral stem impaction, a prophylactic cerclage wire was applied. In a recent report, stabilization of a fissure caused by femoral implant impaction using cerclage wires provided higher resistance to movement and no additional subsidence when compared with intact bone . We do not believe that the potential for fracture complications should be an unduly limiting factor when considering recommendation of THR in juvenile dogs.…”

“…Femoral fissures and their propagation to fractures are a potential complication with cementless femoral stem implantation . The risk factor can be alleviated by neutralizing the forces on a fissure with full cerclage wires . The indication for the use of cerclage wires is the same for Ti EBM and CoCr stems.…”

“…44, 45 The risk factor can be alleviated by neutralizing the forces on a fissure with full cerclage wires. 46 The indication for the use of cerclage wires is the same for Ti EBM and CoCr stems. The full cerclage wires used were precautionary, because a new device was being used, even though no fissures were observed during surgery.…”

“…Treatment of periprosthetic fractures in dogs and potential strategies to prevent fractures have been reported, but mechanical studies have not been performed to test adjunctive fixation . A biomechanical study tested the hypothesis that stabilization of periprosthetic fractures with cerclage wire would restore stem stability after periprosthetic fracture . Femora were implanted with BFX ™ uncemented femoral stems (BioMedtrix, Boonton, NJ) and axially loaded to failure.…”
