Response to highly active antiretroviral therapy among severely immuno-compromised HIV-infected patients in Cambodia

Abstract: Although CD4 cell count-stratified mortality rates were similar to those observed in industrialized countries for patients with CD4 cell count > 50 cells/microl, patients with CD4 cell count < or = 20 cells/microl posed a real challenge to clinicians. Widespread voluntary HIV testing and counselling should be encouraged to allow HAART initiation before the development of severe immuno-suppression.

“…In our study, early initiation of HAART was associated with a lower risk of death, implying the need for a earlier diagnosis of HIV infection through improved counseling and testing practices, including careful implementation of the Prevention of Mother-to-Child Transmission (PMTCT)-plus strategy, and a much earlier initiation of HAART [12].…”

“…Of the nine, seven reported an estimate for the effect of cotrimoxazole on mortality, [28][29][30][31][32][33][34] whereas three reported an estimate for the effect on morbidity. 28,35,36 Two studies also reported the preventive effect on malaria.…”

“…28,35,36 Two studies also reported the preventive effect on malaria. 28,36 Assessment of publication bias indicated that three studies [28][29][30] were of high methodological quality, while the other six [31][32][33][34][35][36] were of moderate methodological quality (Table 4, available at: http://www.who. int/bulletin/volumes/90/2/11-093260).…”

“…Understanding such patients is critical as they may represent individuals who transferred their care to another clinic or individuals who default completely but may later reenter care. Given that men were more likely than women to be lost to follow-up, as was observed in an evaluation of an ART program in Cambodia, 30,31 gender-specific strategies to retain and track patients in HIV care are needed.…”
