Rest left ventricular function and contractile reserve by dobutamine stress echocardiography in peripartum cardiomyopathy

Abstract: Left ventricular ejection fraction at baseline was a predictor of left ventricular functional recovery in patients with peripartum cardiomyopathy. Dobutamine stress echocardiography at presentation of the disease did not predict recovery at follow-up. Myocardial fibrosis appeared to be uncommon in this cardiomyopathy.

“…In a series of seven patients, the presence of inotropic contractile reserve-defined as the improvement of wall motion score in ≥2 segments-was a better predictor than resting LVEF of LV functional recovery during follow-up (49). Conversely, in another cohort of nine patients, resting LVEF was the sole predictor of LV functional recovery at follow-up (50). Small sample sizes, different DSE protocols, and clinical characteristics of the two patient populations may explain the conflicting results.…”

“…Dobutamine is a synthetic catecholamine widely used in the management of stress, cardiogenic shock and HF [17,18]. Dobutamine primarily stimulates β 1 -adrenergic receptors, and partly the β 2 -adrenergic receptors, in the heart to produce dose-dependent ionotropic and chronotropic effects, and is used for maintenance of cardiac functions in severe heart failure [19,20]. Intravenously administered recombinant human brain natriuretic peptide (rhBNP) is the recombinant formulation of human Btype natriuretic peptide, a 32 amino acid endogenous neurohormonal released from the atria and cardiac ventricles in cardiac overload, wall stress and hypertrophy [21,22].…”

“…Currently, no evidence based strategies are available for stratifying risk in women with recovered left ventricular function who wish to conceive again, although some researchers have advocated for the assessment of contractile reserve with stress echocardiography 154155. Women with a history of PPCM should be counseled about the risks of subsequent pregnancy and should be followed closely throughout pregnancy and until six months postpartum with frequent clinical examinations and serial echocardiograms 116151.…”
