Results after endoscopic treatment of postoperative upper gastrointestinal fistulas and leaks using combined Vicryl plug and fibrin glue

Abstract: Postoperative upper gastrointestinal fistulas or anastomotic leaks can be managed successfully with little morbidity by means of endoscopic insertion of Vicryl mesh with fibrin glue, thereby avoiding repetitive major surgery and its associated risks.

“…The use of fibrin glue to seal low output fistulas has been reported in small series, sometimes used in serial injections at the surgeon's discretion [2,[51][52][53]. While results have been largely disappointing, there seems to be some utility in carefully selected patients with favorable fistula anatomy and physiology [2].…”

“…Some authors advocate adding a polyglactin plug (Vicryl, Ethicon Inc.) to enhance closure of fistulas originating in upper gastrointestinal tract. 69 Successful closure of fistula following percutaneous application of histoacryl has also been described in two patients with gastrocutaneous and duodenocutaneous fistula. 70 Lisle et al and Khairy et al described gelatin sponge (Gelfoam, Pharmacia and Upjohn Company, Kalamazoo, MI) embolization of the internal opening using a percutaneous approach for fistula originating from both the small bowel and the large bowel.…”

“…Treatment success with further endoscopic procedures was seen. Vicryl plug or clipping are reported as additional endoscopic procedures because endoscopic treatment failure with consecutive surgical intervention became necessary [15,[19][20][21][22].…”
