AimTo evaluate the effect of cigarette smoking on periodontal health at patient, tooth, and site levels following supportive therapy.Materials and MethodsEighty chronic periodontitis patients, 40 smokers and 40 non‐smokers, were recruited to a single‐arm clinical trial. Periodontal examinations were performed at baseline (T0), 3 months following active periodontal therapy (T1), and 12 months following supportive periodontal therapy (T2). Smoking status was validated measuring serum cotinine levels. Probing depth (PD) ≥ 5 mm with bleeding on probing (BoP) was defined as the primary outcome. Logistic regression analyses adjusted for clustered observations of patients, teeth, and sites and mixed effects models were employed to analyse the data.ResultsAll clinical parameters improved from T0 to T2 (p < 0.001), whereas PD, bleeding index (BI), and plaque index (PI) increased from T1 to T2 in smokers and non‐smokers (p < 0.001). An overall negative effect of smoking was revealed at T2 (OR = 2.78, CI: 1.49, 5.18, p < 0.001), with the most pronounced effect at maxillary single‐rooted teeth (OR = 5.08, CI: 2.01, 12.78, p < 0.001). At the patient level, less variation in treatment outcome was detected within smokers (ICC = 0.137) compared with non‐smokers (ICC = 0.051).ConclusionSmoking has a negative effect on periodontal health following 12 months of supportive therapy, in particular at maxillary single‐rooted teeth.

Las enfermedades periodontales son unas graves infecciones bacterianas que destruyen las fibras de inserción y el hueso de soporte que mantienen los dientes en la boca. Sin tratar, esta enfermedad puede llevar a la pérdida dental (Medical Dictionary). Los estudios longitudinales han centrado su atención hacia la periodontitis crónica. Se ha documentado el decisivo papel de la placa bacteriana en la iniciación y en el mantenimiento de la gingivitis, y que, los efectos dañinos sobre los tejidos y la gravedad de estos efectos están regulados por una compleja interacción entre el parásito y huésped. El tratamiento de la lesión periodontal cumple, para el tratamiento periodontal básico, con el propósito de eliminar y prevenir la recurrencia de los depósitos bacterianos localizados en las superficies dentarias supragingivales y subgingivales y, para el tratamiento quirúrgico con el objetivo de crear acceso para el desbridamiento profesional correcto de las superficies radiculares infectadas y establecer una morfología gingival adecuada que facilite el autocontrol de la placa por parte del paciente. Diferentes técnicas se han utilizado para alcanzar el objetivo de mejorar el pronóstico de los dientes a largo plazo. Desafortunadamente no son muchos los estudios que consiguen demostrar la efectividad de las técnicas utilizadas, con una evolución a lo largo del tiempo dejando entonces algunas incertidumbres.

The objective of this study was to test the hypothesis that the application of a chlorhexidine-based decontamination protocol during two weeks prior to scaling root planing (SRP) would reduce bleeding on probing (BoP) at the time of SRP. Secondary outcomes were other clinical parameters. Another secondary aim was to assess whether the improvement of periodontal conditions, if any, would have a benefit up to 3 months later. In this mono-centre, parallel, single blind, randomised, controlled clinical study, thirty subjects affected by chronic periodontitis were randomly allocated 1:1 to either a control group (n=15, individuals subjected to a standard one-stage full-mouth disinfection procedure fifteen days after inclusion), or a test group (n=15, individuals had to apply a decontamination protocol consisting of tongue brushing and mouth rinsing with chlorhexidine-based products during the fifteen days intervening between inclusion and SRP. BoP, probing depth (PD), plaque index (Pl.I) and clinical attachment loss (CAL) were measured at the inclusion and at t SRP =baseline, when individuals were subjected to SRP, and after 30 and 90 days. Immediately before SRP, BoP was significantly reduced in test (68.31±14.70) with respect to control group (29.54±11.97, p<0.0001). The other clinical parameters (except for CAL) were significantly reduced, with comparable improvements between the two groups after 30 and 90 days. Although investigations on a larger sample are desirable, the early application of a disinfection procedure improves the condition of patients undergoing full-mouth therapy. This leads to marked advantages both for patients, who exhibit reduced bleeding, swelling and pain, and the operator, who is less operatively limited during the hygiene session.
