Results of Spirometry Among Individuals in a Silicosis Registry

Abstract: Both obstructive and restrictive patterns were observed regardless of smoking status with a low profusion category of simple silicosis. There was an increased prevalence of restrictive changes with increased profusion. After controlling for age, smoking, and duration of exposure to silica, there was a statistically significant increased risk of obstructive and mixed changes with progressive massive fibrosis.

“…Similar patterns were also reported by Enfield et al []. This difference may be attributed to a mixed restrictive and obstructive pattern that is common in silicosis, particularly in the presence of progressive massive fibrosis [Rosenman et al, , Leung et al, ]. Other confounders that were not taken into account may also have disadvantaged survival among those with IPF relative to silicosis, masking worse outcomes for the latter (opposite to the pattern we observed), but these would have to be powerful unmeasured risks to be responsible for the observed protective effect of silicosis.…”

“…FVC and FEV 1 values trend down as the disease progresses, accompanied by increased radiologic findings . One study analyzed 526 spirometries of patients with various stages of silicosis and reported an obstructive pattern in 67.2% . On the other hand, another study analyzed patients with advanced silicosis who were on the waiting list for LTx, and found PFTs compatible with severe restrictive conditions.…”

“…As there are currently no criteria to classify end‐stage silicosis, which would call for LTx, we use the current International Society of Heart Lung Transplantation guidelines to define end‐stage disease . Patients who meet the above criteria are placed on the LTx wait list following a high‐resolution chest computed tomography (CT) scan . In this study, we included all patients on the wait list for LTx due to end‐stage silicosis at the ISCMPA between January 1, 1989, and July 31, 2015 (N = 26).…”

Both COPD and lung cancer manifest as irreversible disease courses and further result in poor prognoses among these patients.…”
