Retained Catheter Fragment from a Fractured Tunneled Catheter-A Rare and Potentially Lethal Complication

Abstract: Despite efforts to curtail central vein catheter use for dialysis catheters are frequently used in the treatment of end-stage renal disease (ESRD). In 2006, 82% of patients in the USA initiated dialysis via a catheter. The overall of tunnelled cuffed catheter (TCC) use was 35% greater in 2005 compared with 1996. Dialysis catheter tip fracture is a rare and potentially serious complication. Herein, we present the case of an incidental finding of a retained catheter fragment from a fractured TCC in the right atr… Show more

“…Despite the relative speed of the procedure (15'), Foley et al report incomplete removal of the catheter with a residual fragment left in situ (21). Although no consequences are reported regarding this case, there is still the possibility of long term and potentially life-threatening complications (22).…”

“…Right internal jugular haemodialysis catheter fracture has been reported, and attributed to its repeated manipulation and acute angle in the neck region (site of fracture) 5. In another case involving a haemodialysis patient, the tip of a previous right internal jugular catheter was found in the right atrium, following a CXR for an unrelated problem 6. This patient had previously had multiple venous catheters.…”

“…In this method, a 16‐French laser sheath (Spectranetics Inc., Colorado Springs, CO, USA) passes over the dialysis catheter transecting adhesions. Some reported complications of retrieving a retained catheter include catheter fracture, vessel rupture, and fragmentation of catheter remnants causing uncontrolled bleeding, sepsis, and thrombosis …”
