Abstract

S Reteplase Dissecting intramural haematoma of oesophagus: case reportA 59-year-old man developed dissecting intramural haematoma of oesophagus during treatment with reteplase.The man experienced chest pain and presented to a hospital. He developed ST-elevation myocardial infarction and received a thrombolytic agent reteplase [route and dosage not stated]. Ninety minutes later, an electrocardiography showed a reduction of less than 50% in ST elevation. A coronary angiography indicated a left anterior descending artery cut-off after the large diagonal branch. A rescue percutaneous coronary intervention was performed and a drug-eluting stent was inserted into the left anterior descending artery. An hour later, he experienced a severe compressing chest pain which was accompanied by two episodes of haematemesis. An endoscopy showed a dark, purplish mass bulging into the oesophageal lumen in the upper to lower oesophagus. A chest CT with contrast showed a lesion compatible with oesophageal haematoma.The man received treatment with analgesics and hydration.
