We read the article by Drabeni et al 1 with great interest. The authors determined that immunohistochemical staining for Melan-A identified neoplastic melanocytes at a greater Breslow thickness than appreciated on routine hematoxylin-and eosin-stained sections. We have also used Melan-A to help identify neoplastic melanocytes and have found this technique to be very useful in determining an accurate depth of melanoma invasion. We urge clinicians to be cautious when using Breslow thickness determined with the help of immunohistochemical staining for use in prognostic models. Prognostic models have relied on melanoma data obtained by conventional methods. Current therapeutic guidelines are also based on the examination of hematoxylin-and eosin-stained sections. 2 Use of measurements obtained using immunohistochemical techniques could provide erroneous information when used in prognostic and treatment models designed for measurements obtained by conventional means. Variation of mitotic counts as determined by conventional and immunohistochemical techniques 3 provides one example of how newer techniques may provide differing results from conventional methods. As methods become more refined, clinicians must find a way to integrate data and revalidate accepted models used to determine prognosis and therapy.

Retiform hemangioendothelioma is considered as a low grade angiosarcoma, commonly seen in 2nd-4th decade of life. The youngest patient reported is of 9 years of age. A 9-year-old boy was presented with an asymptomatic red patch on the chest since 1 year. There was an erythematous patch with nodule of about 5 mm near the distal margin. Biopsy from the nodule revealed numerous thin-walled branching vessels in dermis resembling rete testis, suggestive of retiform hemangioendothelioma. We, hereby, report this case for its rarity and uncommon clinical presentation in childhood period.
