Retinal inflammation diagnosed as an idiopathic macular hole with multiple recurrences and spontaneous closures

Abstract: Rationale:An idiopathic macular hole that causes substantial reduction in central visual acuity is believed to involve no obvious underlying diseases; thus, it is suspected to form due to the presence of idiopathic tractional forces at the vitreoretinal interface. Importantly, it is effectively treated with pars plana vitrectomy (PPV), which removes the mechanical forces. However, while it is exceedingly rare, a macular hole can develop in eyes after PPV; fresh or postoperative macular holes can close spontane… Show more

“…Fibrin-mediated retinal damage may be an important cause of retinal degeneration related to subretinal hemorrhage [22]. Another explanation might be that retinal inflammation was associated with the pathogenesis of MH formation as indicated by Hayashi et al [13].…”

“…Secondary MH may develop in association with trauma [2], macular edema [3], retinal detachment [4], choroidal neovascularization [5][6][7], or submacular hemorrhage (SMH) [8][9][10]. Spontaneous closure is a rare event [7,[10][11][12][13][14][15][16]. We present a rare case of the development of secondary MH and spontaneous closure of MH associated with SMH due to polypoidal choroidal vasculopathy (PCV).…”

“…Ocular inflammation can be associated with postoperative MH opening and closure. 10 Since the timing of complete resolution of ocular inflammation and MH closure is similar, the resolution of ocular inflammation might have positively affected spontaneous MH closure.…”

“…An earlier report described an idiopathic MH with multiple recurrences and spontaneous closures that involved retinal periphlebitis, as recorded by FA during MH recurrence, and closure was achieved by topical steroid [ 5 ]. Thus, inflammation could be involved in certain types of MH; the unique point of the affected eye in the current case was that closure was most likely achieved by photocoagulation to the NPA.…”

“…Thus, inflammation could be involved in certain types of MH; the unique point of the affected eye in the current case was that closure was most likely achieved by photocoagulation to the NPA. Moreover, Hayashi et al also reported that a bridging element was found before MH closure [ 5 ]. The development and thickening of the bridging element preceded the recovery of ELM (Fig.…”
