Retrospective Multicenter Survey on Food-Related Symptoms Suggestive of Cow’s Milk Allergy in NICU Neonates

Abstract: We confirmed that the clinical manifestations of food-related symptoms suggestive of CMA in neonates were distinctly different from those of common immediate type food allergy and were largely affected by underlying factors such as prematurity and gastroenterological surgery. Further OFC-based prospective accumulation of cases of CMA in neonates will be particularly important to reveal the full clinical features of this disease.

“…The clinical features repeat every single time the child eats the culprit food, and it is totally resolved once these proteins are removed from the diet . Although several reports have been published in the past few years, there is still a lack of knowledge about this syndrome, its epidemiology, and its clinical characteristics FPIES is thought to be a cell‐mediated disorder but is uncertain about the role of specific IgE in this pathology. It has been speculated that there might be a local IgE production in the intestinal mucosa that participates in reaction to the food but would not reach the blood stream …”

“…His serum level of immunoglobulin (Ig)E was <5 IU/mL, and he was negative for cow's milkspecific IgE. Additional diagnostic tests such as allergenspecific lymphocyte stimulation test and fecal eosinophils were not performed at this time because the falsepositive rate is high in this condition 1,2) . Changes in the profiles of 17 different cytokines including IFNγ are shown in the table 2) .…”

“…Milk-specific IgE is usually not established in newborns with CMA. Therefore, an oral food challenge test should be the main diagnostic method for CMA (3). If the infant has been fed with formula milk, the formula should be changed to an extensively hydrolyzed formula or amino acid formula (4).…”
