“…The non-surgical management of PMD includes spectacles and different groups of contact lenses ( 6 - 8 ), yet these procedures lose their effectiveness as the disease progresses. Patients with high or irregular astigmatism cannot be corrected with spectacles; in these cases, contact lens fitting is more difficult in pellucid eyes than keratoconic eyes as a result of central flattening and peripheral steepening ( 9 - 12 ).…”

“…Most patients with PMD are treated non-surgically with spectacles or contact lenses. 1,9,10,30,31 Soft and hybrid contact lenses can be used in early PMD, but lose their effectiveness Pellucid Marginal Degeneration/Moshirfar et al as the disease progresses. [31][32][33][34][35][36][37][38] In moderate PMD, the development of irregular astigmatism and progressive corneal irregularity often necessitates the use of large diameter rigid gas permeable lenses.…”

“…8,13 In addition, specialized contact lenses, such as reverse geometry lenses and "bi-toric" lenses can be useful in the management of moderate PMD. 14,15 In advanced PMD, the scleral contact lens is a helpful nonsurgical treatment when the fitting with corneal lenses is extremely difficult because of the peripheral location of the ectasia.…”
